Do you accept Medicaid at Evergreen Senior Living?
Yes — we sure do!
Evergreen Senior Living proudly accepts Medicaid for eligible residents through the Illinois Supportive Living Program!
It means wonderful care can be more accessible, and that matters to us. Even better? Our team is happy to walk you through the process, help with paperwork, and cheer you on every step of the way.
And here’s some great news: the Supportive Living Medicaid program is different from traditional community Medicaid, with its own set of guidelines—so even if someone doesn’t qualify for one, they may still be eligible for the other!

What is the age requirement to live in the supportive community?
We’re so glad you asked—this is one of the most common questions families have.
Our Supportive Living Community welcomes adults age 65 and older, which aligns with Illinois’ supportive living guidelines and ensures we’re fully equipped to offer the right level of care.
This age‑specific focus allows us to tailor services, activities, and support to the unique needs of older adults, helping residents feel secure, understood, and truly at home.
What size apartments do you have available and how much do they cost?
At Evergreen Senior Living, we offer both studio and one-bedroom apartments. Learn more about our apartment options on our Apartments Page.
Financial assistance is available for all apartment types. We also accept Illinois Medicaid, which allows us to offer apartments to residents with a wide range of income levels through a sliding scale.
Are apartments furnished or unfurnished?
Residents bring their own furniture, décor, and meaningful items to create a space that feels just right.
You get to bring the pieces you love, making your apartment feel familiar and truly yours from day one. Life at Evergreen is all about feeling at home, and your apartment should reflect you.
What is included in the monthly cost?
So much — and it’s one of the best parts of supportive living!
Your monthly all-inclusive fee includes your private apartment, supportive services as needed, and three delicious homemade meals served restaurant-style each day.
It also includes activities, community life, and a caring team that truly pays attention. We’re always happy to walk through the details so everything feels clear and comfortable.
Are there any extra costs that I might expect?
At Evergreen Senior Living, our rates are all-inclusive—your monthly rent remains the same regardless of the level of care or services you receive.
The only additional charges are for optional amenities, including:
- Cable TV
- Landline telephone
- Purchases from the Evergreen Market
- Services at the beauty salon
What happens if I run out of funds to pay privately?
As the only Supportive Living Community in Streator, we proudly accepts Illinois Medicaid. When a resident’s assets fall below the maximum asset allowance, we’ll help initiate the Medicaid application process. Once approved, your rent will transition to a sliding scale based on your income.
This process is seamless—there’s no need to move or change apartments. Every apartment is available to both private-pay and Medicaid-supported residents.
Do you offer financial assistance?
Yes! As a Supportive Living Community, Evergreen Senior Living accepts Illinois Medicaid. If your income doesn’t meet our private pay rates and your assets are below the maximum asset allowance, we’ll assist you in applying for Medicaid.
Your monthly payment will then be based on your total income, and our dedicated Medicaid specialist will guide you through the entire process to ensure it’s simple and stress-free.
What other types of financial assistance options are available?
In addition to Medicaid, residents may qualify for Veterans’ Aid and Attendance for veterans or surviving spouses, or use Long-Term Care Insurance to help cover supportive living costs. Our team can help guide families through these options.
Do you offer help with medications?
Yes, absolutely — and we’re proud of how thoughtfully we do it.
Medication support is available to help residents stay safe, consistent, and confident each day. It takes the stress out of managing medications and brings families real peace of mind. It’s one of those supports that quietly makes a big difference every single day.
We proudly partner with Green Tree Pharmacy to ensure accurate, reliable service and smooth coordination.
Is assistance available with bathing and daily routines?
Absolutely — and we love making this as comfortable and easy as possible! Our team can help with bathing, dressing, grooming, and daily routines, with as much or as little support as each resident needs.
We’ll set up the shower, get the water to just the right temperature, and have a fresh towel and robe ready and waiting. Need help washing your hair or a little extra support? We’ve got you!
Everything is done with kindness, privacy, and full respect for each resident’s comfort level. You’re in good hands here!
Do you provide transportation?
We sure do — and it’s a big hit with residents!
Evergreen Senior Living offers senior-friendly transportation with a lift for medical appointments, errands, shopping trips, and fun outings.
It’s all about staying active, connected, and independent without worrying about driving.
Do you allow pets?
Yes! At Evergreen Senior Living in Streator, we warmly welcome pets. Each resident may have one pet in their apartment, as long as they can fully care for it on their own—including feeding, grooming, and managing bathroom needs.
Can family members visit and be involved in care decisions?
Yes — always, and we truly mean that.
Family members are welcome to visit, stay connected, and be involved in care decisions with the resident’s permission.
Open communication is important to us, and we love working together as a team. Life at Evergreen is better when families feel included and informed.
Do residents get to maintain their independence?
Yes! That’s our goal!
Evergreen Senior Living is designed to support independence while offering help when it’s needed. Residents stay in control of their day, their routines, and their choices.
We’re here to provide backup, encouragement, and support — not take anything away.
Can we have guest stay over?
Yes — guests are welcome!
We love when residents have friends or family visit and feel at home hosting them.
For extended stays, we ask that you check in with our Executive Director so we can ensure everyone stays comfortable.
Sharing life with the people you love is important.
Do I have to sell my car?
No — that decision is completely up to you.
Some residents continue driving as long as they feel comfortable and have a valid license.
Others prefer to use our transportation services instead. Either way, Evergreen supports whatever helps you feel most confident and independent.
Can I manage my own medication?
Yes — and we love supporting independence like that.
If you’re able and prefer to manage your own medications, we fully support that choice.
If your needs ever change, our team is right there to adjust with you.
Life changes, and we’re here for all of it.
Is the building secure at night?
Yes — safety and peace of mind matter to us.
In the evening, the building is secured with a second set of locked doors. If someone arrives after hours, staff are available to help them in.
Families and residents appreciate that extra layer of security while still feeling welcomed and cared for.
